Together with Dharah Gibinj, Community Health, and Northern Rivers Area Health a monthly ‘Diabetes Complications and Assessment Clinic’. This rotates through Casino, Tabulam, Muli, and Coraki, each community hosting three clinics per calendar year.
These clinics consist of a Specialist, GP, Ophthalmologist, Podiatrist, Diabetes Educator, Dietician, Exercise Physiologist, Pharmacist, Pathologist, Aboriginal Health Workers and Registered Nurses. Point of care testing for HbA1c, Urine, BSL, and Lipids screen is done on the day. This clinic gives the client a ‘one stop’ diabetes ‘annual cycle of care’ and important access to specialist health professionals. This provides assessment and treatment of their diabetes and early detection and prevention of co-morbidities.
Ongoing reviews and patient contact through out the year ensures that our clients with diabetes are well monitored. Our aim is to close the health gap.
At present we have diabetes education in the form of meetings and get togethers with Aboriginal Health Workers, an Exercise Physiologist and also visiting speakers on various health topics. These visiting speakers are sometimes an RN, Specialist Physicians or Allied Health Professionals.
