Kidney Program - developed and running

Chronic Kidney Disease has 5 Stages

Our program targets patients with stage 2 and 3. A Renal Nurse Practioner conducts clinics every even month on the 2nd Tuesday. Referrals to these clinics are from regular Health Check screening that has shown abnormal results pertaining to kidney function.

Once a patient attends these clinics they are monitored closely to reduce their progression to the next stages of kidney disease.

Patients who are identifies as stage 4 are referred to a predialysis clinic and are closely monitored by the renal team in Lismore.

All patients with Chronic Renal Disease are identified on a renal register and reviewed regularly.

At present we have renal education in the form of meetings and get togethers with Aboriginal Health Workers, an Exercise Physiologist and also visiting speakers on various health topics. These visiting speakers are sometimes an RN, Specialist Physicians or Allied Health Professionals.
