NB Vascular health is still running as a component of the Chronic Health Program and is being transitioned into the Flinders Model of Care
Vascular patients at this present time are managed through our GPs and chronic health team
At present we have vascular health education in the form of meetings and get togethers with Aboriginal Health Workers, an Exercise Physiologist and also visiting speakers on various health topics. These visiting speakers are sometimes an RN, Specialist Physicians or Allied Health Professionals.
A new vascular health program will be launched in 2009.
A project steering committee will continue to support and oversee both the CV and the PHC Network and will include relevant stakeholders:
